1.報名資料請詳細填寫，英文姓名部份若有護照者填寫護照上英文姓名。身分證欄位外籍生可填寫護照號碼或居留證號碼。
Please fill out the form in detail. If you have a passport, please fill in your English name on the passport. Foreign students can fill in their passport number or residence permit number in the ID card field.

2.報名完後於考試日期十日前至餐旅管理學系辦公室繳交報名費，逾期取消報名資格

After completing the form, please pay the fee to the Department of Hospitality Management office 10 days before the exam date. Late payments will result in the application being cancelled..

3.無正當理由缺考者恕不退費。

No refund will be given for no-shows without a valid excuse.

4.廚藝系同學只能報考Excel電子試算表--實用級

Students of the Department of Hospitality Management only register for the Excel Spreadsheet — Practical Level exam.

5.該梯次如果報名人數少於三十位，將不舉辦。
 If the number of participants for this session is fewer than 30, it will be canceled.
